Application Requirements

The following must be completed in order for the application to be considered:

Complete a student profile through UCSB PaCE.
Applicants must have a high school diploma, GED, or working towards an undergraduate degree from an accredited institution (unofficial transcripts are acceptable for initial application).
Applicants must complete the application form.
Math, Reading, Writing Proficiency as determined by a standardized nationally recognized examination (HESI). A copy of the email sent by Prometric, confirming your HESI exam appointment date and HESI exam scores.
Comprehensive resumé
3 letters of recommendation. One letter must be from your current supervisor (if employed).
Emergency contact information
Submission of an essay including a personal statement describing the candidate's objectives for applying to the Program, including educational and professional goals, as well as professional or personal qualities that will help the candidate succeed in the Program and in the profession.
Applicants must be able to work in the United States
